This Privacy Policy governs and explains how Quality Reviews, Inc. (collectively “Quality Reviews”, “we” and “us”) collects, uses, shares and protects information obtained from visitors to Quality Reviews’ websites located at or and/or the software applications RateMyHospital®, Q-ReviewsSM, Q-RoundingSM, Q-RemindersSM, Q-PROMSSM for mobile devices that are made available by Quality Reviews and that reference this Privacy Policy (such web sites and mobile applications, along with all content and services made available through such site and applications are referred to, collectively, as the “Solution”). The Solution includes survey questionnaires for hospital patients to rate their experience in a hospital they have recently visited on such matters ranging from the cleanliness of the hospital to wait times in the doctor’s office.

When you share information with us, for example by completing a questionnaire via the application or otherwise visit or utilize our Solution, you have agreed and are additionally subject to the Terms of Service, which incorporates this Privacy Policy by reference. As such, you consent to the collection and use of your information under the terms of this Privacy Policy.

Information We Collect

We collect information that you voluntarily provide to us, [including but not limited to your name and contact details,] in the following ways:

    • Information you give to your healthcare provider. We collect the personally-identifiable information (“Personal Information” that you voluntarily elect to provide to your healthcare provider when registering for healthcare services and/or when accessing and using healthcare services. We use your Personal Information to process and administer details of your registration, or, if you are party to a Subscription Agreement (as defined in the Terms of Service) your membership, to communicate with you, to provide you with opportunities, and to keep you up to date with new developments at Quality Reviews. We also use and analyze the information we collect (including your Personal Information) so that we can administer, support, improve and develop our business.

Quality Reviews may contact you using the Personal Information you voluntarily provide.

Quality Reviews is committed to keeping your Personal Information confidential. Quality Reviews does not sell, license, lease or otherwise disclose your Personal Information to any third party except as described in this Section or as required by law, or if we otherwise obtain your prior consent.

Quality Reviews, working collaboratively with the American Nurses Credentialing Center (ANCC), created a crosswalk of Quality Reviews’ questions to the nine patient satisfaction categories (see below) delineated in 2014 and 2019 Magnet® Application Manuals. This crosswalk was approved by ANCC in November 2017.
